This form is for LUSU affiliated student groups and communities (Societies, Sport, Liberation, JCR, Academic Reps.)
If you are organising activity that is not covered by your groups general risk assessment and code of practice, then you need to use this form to tell us about it. This is so we can ensure your activity is supported and has the appropriate provision in place to keep you safe.
You need to tell us about activities or events that will require a specific risk assessment at least 1 month beforehand to ensure we can support you with this in good time.
Please note - low risk activities may not require a risk assessment or to be registered with us. This would include low-level social activity such as meetings in bars, seminar rooms/LTs and restaurants on campus and in town. Please refer to the Student Groups General Code of Practice for guidance and to help you understand if a risk assessment and additional support from the SU will be required.
